of the UOB Property Loan – Introduce A Friend Programme* ("Terms and Conditions") and all of the terms in this UOB Property Loan - Introduce A Friend Programme Form ("Form"); and
to be bound by the prevailing Terms and Conditions and all of the terms in this Form and to participate in the Programme (as defined in the Terms and Conditions);
confirm that all the information which are set out in this Form are all true, correct and complete;
confirm, represent and warrant that I have obtained consent from my friend or loved one named in this Form ("
Third Party
"):-
to refer such Third Party to United Overseas Bank Limited ("
UOB
") for a UOB Home Loan; and
for me to use, collect, obtain and disclose his/her contact details and Personal Details to UOB and for UOB to use, collect, obtain and disclose the Third Party’s contact details and Personal Details in order for UOB to contact him/her to promote UOB Property Loans and in connection with UOB Property Loans;
and that I am authorized by the Third Party to do all of the above. For the purposes of this Form, the term "
Personal Details
"
refers to any data about an individual who can be identified from that data such as that individual’s name, NRIC, passport or other identification number, telephone numbers, address, email address and any other information relating to individuals, which I have provided to UOB;
agree, consent and authorise UOB to use, collect and disclose any of my personal information for the purposes of the Programme and/or UOB Property Loan and/or informing the Third Party that I have referred him/her to UOB for UOB Property Loans;
confirm that I have informed the Third Party of the Terms and Conditions and the fact a reward may be given to me in connection with this referral if I am eligible to receive the reward
irrevocably and unconditionally undertakes:-
not to discuss with or explain to or agree or disagree with or initiate or enter into any such discussion or explanation or agreement or disagreement with or advise any party on any matters in relation to or in connection with any credit or banking facilities products and/or packages or any of its features offered by UOB including, but not limited to, the pricing package terms and conditions of the said facilities, the repayment details and amounts, the interest rate, fees and charges payable; and
to immediately notify such persons that he/she should seek clarification from UOB on the same; and
I shall fully indemnify UOB and keep UOB harmless and fully indemnified from and against all and any claims, actions, liabilities, demands, expenses, losses, costs, charges and damages which UOB may suffer, incur and/or sustain as a result of any representation and warranty turning out to be inaccurate or as a result of a breach of any of the above or any of the Terms and Conditions or and UOB shall be entitled to claim from me and I shall refund to UOB on demand all sums equal to the value of the applicable reward that is given to me in relation to such referral.
